Integrated Care for Kids Model

The Integrated Care for Kids (InCK) Model, developed by CMS, aims to improve care for children covered by Medicaid and CHIP by integrating health and social services through coordinated care and case management. The model focuses on early identification of health needs, reducing avoidable hospital stays and out-of-home placements by addressing both medical and social factors.
